Tender Overview
The National Health Laboratory Service (NHLS) invites qualified service providers to submit quotations for the supply and delivery of LED fluorescence equipment at NHLS Kroonstad. This is a once-off procurement aimed at enhancing diagnostic pathology laboratory services.
Scope of Work
The successful bidder will be responsible for:
- Supplying LED fluorescence equipment that meets NHLS specifications.
- Delivering the equipment to NHLS Kroonstad.
- Providing all labour, materials, accessories, and related components required for the delivery.
- Supplying product brochures or catalogues with detailed technical specifications.
- Including all costs related to delivery, installation (where applicable), supervision, and commissioning.
- Ensuring compliance with all technical and quality standards specified in the RFQ.
Minimum Requirements
Bidders must submit the following documents:
- Valid SARS Tax Clearance Certificate.
- Valid B-BBEE Certificate or Sworn Affidavit.
- Central Supplier Database (CSD) Full Report (dated within the RFQ period).
- CIPC Company Registration Certificate.
- Official quotation.
- Fully completed and signed SBD 4 (Bidder’s Disclosure).
- Fully completed and signed SBD 6.1 (Preference Points Claim Form).
- Fully completed Pricing Schedule and signed SBD 7 (Form of Offer).
- Proof of business address (Municipal Rates Account or Lease Agreement where applicable).
- Fully completed and signed RFQ document.
Technical Requirements
The successful bidder must:
- Supply LED fluorescence equipment that complies with NHLS technical specifications.
- Provide detailed product brochures or catalogues.
- Deliver the equipment within the required timeframe.
- Include all associated delivery, installation, and commissioning costs where applicable.
